Our Purpose: To enrich the quality of life for persons living
with Alzheimer’s disease and related dementias and their care partners—
celebrating and inspiring innovative practices of care and sharing these
practices with the wider dementia care community.
The Awards: Up to five annual awards of $5,000 to individuals who have created meaningful opportunities in one of four categories of care to improve the quality of life for persons living with dementia and care partners. For full details, visit the Awards page and read the Awards FAQ.
Who May Apply: Individuals (including but not limited to health care professionals and care partners) located in the United States, who have created meaningful opportunities to improve the quality of life for persons living with dementia and care partners. Your program or innovation must have been active in 2026 or the year prior. Please note: you make a single application as either an Individual or Organization application, not for both.
Note: The term “care partner” refers to family members, friends, domestic partners, and neighbors who may be paid or unpaid.
